[Danhi] Five Star Club/Bright Star Club

I am starting a 5 Star Club and Bright Star Club for second semester

Students need to complete all five requirements to get in the club.  The
Bright Star Club, is the highest leveled club. Students can always get a
chance to move up to the Bright Star Club, once they have acheived all the
requirements for the 5 Star Club. Please practice with your students over
the holiday break! I will begin testing for memberships to our clubs on
January 8, 2013

5 Star Club Requirements:

Can I write my name legibly?

Do I know my birthday/including the year?

Do I know my address?

Do I know my phone number?

Can I tie my shoes?

Bright Star Club:

Can I write my first, middle (if they have) and last name legibly?

Do I know my birthday/including year and time of my birth and how much I
weighed?

Do I know my address? ( Must include zip code, city and state?

Do I know my phone number/ including area code?

Do I know an emergency contact phone number? 

Do I know how to tie my shoes?
